Thank You for choosing Naresh Rana MD for your health care needs. The following information is designed to help you become more aquatinted with Dr. Rana's practice. If you have any questions, please do not hesitate to ask.

If you are a new patient visiting Dr. Rana's office, we ask that you arrive 15 minutes prior to your visit to complete all appropriate paperwork, or you may download, print and complete our patient forms below and bring them with you to your appointment.

Also, we ask that you bring a photo ID, insurance cards and any other information supplied by your insurance company.

All prescription refill requests require an office visit with Dr. Rana. Please supply the office with an updated formulary list from your insurance company and remember to bring all your current prescription medication bottles (with attached pharmacy labels) to each appointment.

The doctor's staff will attempt to determine the lab or facility that your insurance covers, but it is ultimately your responsibility as the patient/insured to confirm the correct lab or facility. The majority of insurance companies require us to utilize Lab Corp or Quest Diagnostics for conventional lab work. All results from these labs take at least seven working days to be received by us. Once your results are available, they will be reviewed by Dr. Rana. Unless other arrangements are made, all discussions regarding lab results will be made with the doctor during a follow-up appointment, including any medication or dietary issues that need to be addressed.

Payment is due at the date of service. Your payment may consist of an insurance co-payment, deductible, co-insurance, or full payment of $85.00 for uninsured patients or those services not covered by an insurance plan.

Dr. Rana's office accepts Medicare. The following commercial insurances are accepted:

As a courtesy, we will bill your insurance company for you. If there is a difference between what they pay and the actual bill, you will be expected to pay the balance to Naresh Rana MD PA.
